How New Acne Drugs Are Developed

The U.S. Food and Drug Administration (FDA) states its purpose in the following mission statement:

“The FDA is responsible for protecting the public health by assuring the safety, efficacy, and security of human and veterinary drugs, biological products, medical devices, our nation’s food supply, cosmetics, and products that emit radiation. The FDA is also responsible for advancing the public health by helping to speed innovations that make medicines and foods more effective, safer, and more affordable; and helping the public get the accurate, science-based information they need to use medicines and foods to improve their health.”

In 2005 the FDA provided a public draft guidance document to the pharmaceutical industry on the development of drugs to treat acne vulgaris, or acne. The draft guidance document contains the FDA’s current thinking on the development of acne drugs and the methods that should be used.

Some of the guidance document information is summarized below. At the time of this writing the FDA document had not been finalized.

Types of Acne Lesions

The two major types of acne lesions are classified as non-inflammatory and inflammatory. Non-inflammatory acne lesions are more commonly known as whiteheads and blackheads. Inflammatory acne lesions include papules and pustules and are more deeply seated in the skin than the non-inflammatory lesions.

How Acne Severity Is Rated

Currently there is no standardized method for rating the severity of acne outbreaks. Several methods have been proposed, but each has its difficulties. A sample scale for rating acne severity is summarized below, from least to most severe:

0 Clear skin with no inflammatory or non-inflammatory lesions

1 Almost clear; rare non-inflammatory lesions with no more than one small inflammatory lesion

2 Mild severity; greater than Grade 1; some non-inflammatory lesions with no more than a few inflammatory lesions (papules/pustules only, no nodular lesions)

3 Moderate severity; greater than Grade 2; up to many non-inflammatory lesions and may have some inflammatory lesions, but no more than one small nodular lesion

4* Severe; greater than Grade 3; up to many non-inflammatory and inflammatory lesions, but no more than a few nodular lesions

(* worsening beyond Grade 4 is possible).

Acne Drug Treatment Study Groups

Acne drug test groups should be large enough to be statistically significant so that any claims of success and safety can be supported. Test patients are usually recruited during their most severe acne breakouts.

Acne drug trials are recommended to be randomized, blinded, multicenter trials, and other requirements will also apply. A control or placebo group should be utilized.

Furthermore, those doing testing should recruit test populations that are representative of the age, race, gender, and geographic location of acne patients in the United States.

Determining The Success Of Acne Drug Treatments

Acne severity is rated at the beginning and end of every test. Photographic evidence and/or lesion counts may also be taken. It is proposed to measure the success of acne medications as either a success or a failure, based on one of these two methods of measuring success:

1. Success meaning a rating of clear skin or almost clear (severity rating 0 or 1) within the test period; or

2. Success meaning an improvement of 2 severity grades within the test period. Under this definition, a successful test subject would have a severity improvement from 4 to 2 within the test period, as an example.

The Final Decision On New Acne Drugs

Several phases of testing are recommended for proposed acne drugs. Studies must be designed to account for effects such as test dropouts. Rigorous data analysis must be performed in conformance with FDA regulations. Only acne drug test data that has been validated and undergone quality assurance testing should be submitted to the FDA for approval.

Only through a rigorous and tightly-controlled testing and analysis procedure, followed by FDA approval, can new acne drugs be made available to the public.

Beauty

There are many vantage points from which to find beauty. The side of the mountains overlooking Lake Louise on the grass of the hotel sipping champagne with your most cherished friend is a memory of beauty that will always overcome my consciousness. The purr of my friend and companion as he welcomed me home from a hard days work or the smile on the face of most any person who ’sees’ what little part of life we can partake in to help each other. There are many beauties and no real evil. This will be dealt with in greater detail under Abraxas in the concept segment. In the whole of the ‘brotherhood’ there are many religions and in each one; I have found the essence of beauty expressed by wise and metaphysical or soulful people. In Islam my favorite is Sufism. They make a good point about the poison of negativity and any form of thought that does personal energized harm to other life in this quote:

“The soul who had to sympathize with the whole world was thus prepared that the drop of that poison which always produces contempt, resentment and ill feeling against another was destroyed first. So many talk about purification of the heart, and so few really know what it is. Some say to be pure means to be free from all evil thought, but there is no evil thought. Call it evil or call it devil, if there is any such thought it is the thought of bitterness against another. No one with sense and understanding would like to retain a single drop of poison in his body. And how ignorant it is on the part of man when he keeps and cherishes a bitter thought against another in his heart. If a drop of poison can cause death of the body, it is equal to a thousand deaths when the heart retains the smallest thought of bitterness.” (56)

The fear of nature or one’s inner uncertainties has caused many to find evil in places where beauty exists. Once one no longer has to control nature, there is no place where beauty will not be found. It may be a bittersweet beauty such as seeing your lover find a person she wants to marry; when you are an older man with all the love in the world for a person you love in every way. It may be the joy you feel when you know you aren’t going to have to schlep or drudge for money in the socially structured gambit to gain material things. The fears that create the opposite of harmony and brotherhood are the ‘poisons’ which no true Kelt would know as he/she spoke to Alexander about their creed and their honest appreciation for the soul in everything. Nature has no evil and the crooked bristlecone pine that dormantly awaits the desert rain has lived longer than the rash fast-growing ash that sprouts quickly skyward. In the soul that knows it is immortal the idea of change is welcome and thus the warrior’s death is too.

Thus one who truly loves will let all things and thoughts go free and hope to share the beauty of ‘what is’. The Cathars had great ‘brothers’ in the Sufis who few people in the world are ready to accept the wisdom of in that war-torn realm of misogyny we call the Middle East. In my limited appreciation for the beauty of Jesus I see him being the kind of person who learned and changed throughout each day and therefore all the time he spent on earth. Each moment is a ‘Zen’ opportunity for the ‘greater good’. But how many of our ways of organizing social behavior really wish good for each individual? There is no beauty in more of any THING; rather beauty exists in the fullness of each and EVERY thing. Oh yes, the turn of phrases and eloquence that waxes and wanes poetic can be a joy which in the crevices of thought are beauty-full but are they really beauty when they seek to arrange MORE for the politician and his cronies or platform? Alexander is said the first to have declared himself Divine while alive. We are in truth, all Divine. It is a divine comedy to be sure, this thing we call life.

The skin is the biggest organ in our body and absorbed whatever we put on it. Nature Healing Method for Acne

The treatment of acne by the administration of salve or ointment does not serve any purpose. They only suppress the action of the sebaceous glands temporarily. In nature cure, the main emphasis is on diet and certain water applications.

To begin with the patient should resort to all fruit diet for about a week. In this regimen, there should be three meals a day, consisting of fresh juicy fruits, such as apples, pears, grapes, grape-fruit, pineapple and peaches. Citrus fruits, bananas, dried, stewed or tinned fruits should not be taken. Unsweetened lemon or plain water,
either hot or cold, should be drunk and nothing else. During this period, warm -water enema should be taken daily to cleanse the bowels and all other measures adopted to eradicate constipation.

After a week of all fruit diet, the patient can gradually adopt a well-balanced diet. Emphasis
should be on raw foods, especially fresh fruits and vegetables, sprouted seeds, raw nuts and
whole grain cereals, especially millet and brown rice. Further shorter periods on the all-fruits for
three days, or so may be necessary at a monthly interval till the condition of the skin improves.
Strict attention to diet is essential for recovery. Starchy, protein and fatty foods, should be
restricted. Meats, sugar, strong tea or coffee, condiments, pickles, refined and processed foods
should all be avoided, as also soft drinks, candies, ice cream and products made with sugar and
white flour.

Two vitamins, namely, niacin and vitamin A have been used successfully to treat acne. The
vitamin therapy which may comprise a niacin, 100 mg. three times daily and vitamin A in large
doses upto 1,50,000 units per day should not exceed one month. Vitamin E is also vitally
important to prevent scarring from acne and in removing old scars.
Another effective remedy in the realm of nutrition that seems to offer new promise of help for
acne is zinc. It has shown dramatic results in some cases. Zinc should be taken in therapeutic
doses of 50 mg. three times a day. After noticeable improvement it can be gradually reduced.

Local Treatment

As regards local treatment, hot fomentation should be applied to open up the pores and squeeze
the waste matter. Then rinse with cold water. Sun and air baths by exposing the whole body to
sun and air are highly beneficial. The healing packs made of grated cucumber, oatmeal cooked
in milk, and cooked, creamed carrots used externally, have been found to be effective.

The orange peel is valuable in the treatment of acne. The peel, pounded well with water on a
piece of stone, should be applied to the affected areas. The lemon has also proved beneficial in
removing pimples and acne . It should be applied regularly.
A teaspoonful of coriander juice, mixed with a pinch of turmeric powder, is another effective
home remedy for pimples and blackheads. The mixture should be applied to the face after
thoroughly washing it, every night before retiring.

The juice of raw potatoes has also proved very valuable in clearing skin blemishes. This
cleansing results from high content of potassium sulphur, phosphorous and chlorine in the
potato. These elements are, however, of value only when the potato is raw as in this state they
are composed of live organic atoms.

A hot Epsom-salt bath twice a week will be highly beneficial in all cases of acne. This bath is
prepared by adding one and a half kg. of Epsom -salt to 50 litres of water having a temperature
of about 100 o F. The patient should remain in the bath from 25 to 35minutes till he perspires
freely. After the bath the patient should cool off gradually.
